PURPOSE OF JOB:
To use skills, knowledge and understanding of practice within the area of specialism to support and deliver in the development of service provision by:
Providing assessment, care management and social work services to service users and carers arising out of the council’s duties and powers under legislation, council and group policy.
To make appropriate use of local, departmental and independent resources.
To work in accordance with guidelines set out by management.
Maintain high standards of professional practice within your own workload and to promote and enhance such practice within the team.
To undertake assessments within field of a general hospital specialism.
To carry out assessments and to undertake care management in the area of specialism in accordance with Group eligibility criteria.
To provide a social work service to designated service users.
To manage, monitor and review care plans, involving users and carers and liaising with other workers and agencies as appropriate.
To work at all times within the Council’s Financial Regulations.
To be aware of the resources available within the specialist area, including other sources of funding so that the maximum benefit may be obtained for service users within budgetary resources of the Group.
To comply with the Group’s recording policy, and to complete appropriate forms and documents as required.
To make use of management information systems, including information technology, in the recording, retrieval and analysis of information as required by the department.
To be managed by and receive supervision from the immediate line manager and to be assessed against agreed standards and targets.
To undertake training as required.
To work as a part of a team, participating in the duty system.
At all times to carry out those responsibilities with due regard to the Council’s Equal Opportunities/Race Equality and Health and Safety Policies as an integral part of the post.
To undertake any other appropriate duties commensurate with the post as directed by the Team Manager or Services Manager.

To monitor and review services and goods purchased, to ensure that they meet appropriate standards, are value for money, meet the requirements of the Group and adhere to financial regulations.
To implement the Council’s financial polices, procedures and practices within the area of specialism. To contribute to training programmes and to provide Training and Development opportunities within the Social Services Group concerned with areas of specialist work in collaboration with Staff Development and Training
In particular to undertake the following: –
A) To assess the needs of Older People who are in hospital or in the community who may require Community Care Services, on request, and to provide Care Plans and arrange for the provision of services where the Groups Eligibility Criteria are met.
B) To investigate alleged neglect, ill treatment or abuse of Older People and to make enquiries as to the need to provide services subject to Group eligibility criteria.
C) To undertake the preparation of reports, statements and other such legal documents as may be required in relation to Guardianship applications (under the Mental Health Act 1983) applications to the Court of Protection and applications under Section 47 of National Assistance Act 1948.
To work alongside Health Service colleagues to facilitate safe and timely hospital discharges.
1. To participate in the development and management of systems associated with team functions, including gathering and analysing of information.
2. To work jointly with the Administrative Officers in seeking to improve the knowledge and practical applications of ESCR amongst frontline operational staff.
3. Any other duties appropriate to the post and grade and maybe required to work at other locations.
UK Recognised Social Work Qualification – eg Diploma in Social Work or CQSW. Registration as a social worker with the General Social Care Council.
2.1 Experience of Working as a social worker with Adults in Care Management and Planning.
2.2 Some experience as a Social Care worker in a hospital setting.
Key Legislation practices and responsibilities of the Local Authority including the NHS and Community Care Act 1990, the Community Care (Delayed Discharge) Act 2003 and the Mental Capacity Act 2005.
